About the Role

We are seeking a skilled and compassionate LPN to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support in the comfort of their own homes.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN, in compliance with state and federal regulations, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans to promote optimal health outcomes.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress, including visit notes, medication administration, and other relevant information.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
