Responsibilities:

  • Develop, review, and revise plans of care in collaboration with our interdisciplinary team.
  • Provide nursing care in accordance with physician's plans of treatment and individual plans of care.
  • Observe, record, and report reactions to treatment and changes in client condition to appropriate personnel and/or physicians.
  • Provide ongoing instruction and supervision to clients, families, or other health team personnel in the plan of treatment.
  • Monitor and record patient condition and document provided care services via electronic charting.
  • Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.

Requirements:

  • Active Licensed Practical Nurse or Registered Nurse License.
  • Problem-solving skills and ability to multi-task.
  • Excellent communication skills.
  • Positive, professional attitude.
