We're seeking a compassionate and skilled LPN or RN to join our team as a Private Duty Nurse. As a member of our team, you'll have the opportunity to make a meaningful difference in the lives of our clients and their families.
Responsibilities:
- Develop, review, and revise plans of care in collaboration with our interdisciplinary team.
- Provide nursing care in accordance with physician's plans of treatment and individual plans of care.
- Observe, record, and report reactions to treatment and changes in client condition to appropriate personnel and/or physicians.
- Provide ongoing instruction and supervision to clients, families, or other health team personnel in the plan of treatment.
- Monitor and record patient condition and document provided care services via electronic charting.
- Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.
Requirements:
- Active Licensed Practical Nurse or Registered Nurse License.
- Problem-solving skills and ability to multi-task.
- Excellent communication skills.
- Positive, professional attitude.
