Job Opportunity

The VNA & Hospice of the Southwest Region Inc. is seeking a skilled Physical Therapist to join our team in providing high-quality patient care in a home health care setting. As a key member of our clinical team, you will be responsible for evaluating, planning, and administering physical therapy treatment to patients in their homes, with a focus on pediatric rehabilitation.

Key Responsibilities:

  • Conduct comprehensive physical therapy assessments of patients, including psychosocial, educational, safety, and risk factor parameters, to develop personalized treatment plans.
  • Develop and implement treatment plans to promote optimal functional independence and safety, in accordance with federal and state regulations, organizational policies, and standards of care.
  • Provide skilled physical therapy treatment, including therapeutic techniques, patient/caregiver education, and assessment of patient status and progress.
  • Collaborate with other clinical team members, including physicians, nurses, caregivers, and students, to ensure optimal patient care and communication.
  • Supervise and direct clinical activities of physical therapy assistants, students, and home care aides, in accordance with federal and state regulations, departmental policies, and standards of care.
  • Maintain accurate and complete clinical records, including documentation of patient care and outcomes, in accordance with federal and state regulations, organizational policies, and standards of care.
  • Participate in in-service training programs for customers, including rehabilitation, medical, and nursing staff, to promote best practices and quality care.
  • Engage in marketing and promotion of rehabilitation services, including developing specialized areas of service and participating in public health fairs and medical community events.
  • Collaborate with quality improvement staff to maximize accuracy of clinical notes and OASIS documents.
  • Follow agency protocol with McKesson processes, including locking-down visits, follow-up notes, and visit completion.
  • Coordinate safe patient discharges and transfers, ensuring all necessary paperwork and patient information is processed.

Qualifications:

  • Degree in physical therapy required.
  • Current Vermont State PT license required.
  • Valid Driver's License.

Experience:

  • Two years of clinical physical therapy experience preferred.
