ACT Independent Living Skills Specialist

As a member of the Lifewell ACT Team, you will provide customized community-based services for individuals living with a serious mental illness who require intensive wrap-around services to maintain their independence and avoid hospitalization, incarceration, or crisis situations.

As an ACT Independent Living Skills Specialist, you will work as part of a clinical team to assess client needs, develop individual service plans, and act as a client advocate and liaison with families and community agencies. You will be responsible for providing on-call crisis intervention services, weekend and holiday coverage, and supporting clients in acquiring independent living skills through direct training or collaboration with other team members.

You will serve as a subject matter expert for independent living skills development, identifying and developing programs to support clients in areas such as household maintenance, financial management, and budgeting. Additionally, you will assume primary contact responsibility for a percentage of clients to ensure accurate and thorough maintenance of their medical records.

Qualifications
  • High School Diploma/GED and three years of behavioral health work experience; OR
  • Associate's degree in a behavioral health-related field and two years of behavioral health work experience; OR
  • Bachelor's degree in a non-behavioral health-related field and one year of behavioral health work experience; OR
  • Bachelor's degree in a behavioral health-related field
  • One year of related work experience providing independent living skills in a behavioral health setting
  • Obtain a Fingerprint Clearance Card
  • Current Arizona Driver License and a current satisfactory 39-month Motor Vehicle Record (MVR)
  • Must be 21 years of age
  • Background Check
  • Minimum 1 year recovery from drugs and/or alcohol
  • Minimum typing speed of 25 wpm
