Responsibilities- Conduct thorough health assessments, reviewing medical history, medication adherence, social environment, and other risks.
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care.
- Develop and implement personalized care plans to address identified health risks and improve patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Master's degree in Nursing or Physician Assistant studies.
- Current RN and NP/PA licensure in the state of practice, with prescription authority.
- Board certification in a Matrix-approved specialty.
- Minimum 1 year of experience as a Nurse Practitioner or Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills.
- Competitive pay rate of $95.00 per completed assessment.
