As a key member of our team, the Outreach Case Manager will play a vital role in delivering high-quality services and supports to individuals in the Blackstone Valley area.
Key Responsibilities- Develop and implement individualized service plans to promote independent living skills and self-advocacy.
- Link individuals to community resources and facilitate access to necessary services and supports.
- Facilitate the development of valued community roles and relationships to enhance social connections and community participation.
- Implement medical and medication protocols, and provide crisis intervention as needed.
- Assist individuals with transportation and access to community resources.
- High School Diploma or equivalent, with alternative education/experience considered.
- Valid Driver's License and acceptable driving record.
- Reliability, willingness to learn, and openness to new opportunities.
