Job Summary

This full-time role as a Field Medical Assistant (Field Care Coordinator) will support patients outside traditional clinics – at home and over the phone after hospital events and routine primary care visits. Reporting to the Manager, Tele-Assist Operations, this position involves facilitating telehealth visits with providers in patients' homes, building trust, and rapport. You'll perform clinical assessments, review medications and care plans, document patient information, and connect them with community resources. This may also involve teaching patients and family members to use technology for care coordination.

Key Responsibilities:
  • Facilitate telehealth visits in patients' homes, ensuring providers have complete views of living situations and patients understand care plans.
  • Conduct standard patient assessments and documentation to facilitate complete visits by treating providers and ensure patients understand their care plans.
  • Clinical duties may include providing best-in-class telehealth setup services, performing non-blood specimen collection, spirometry, retinopathy, EKGs, and vitals.
  • Coordinate care with patients, families, and other outside care teams as needed, connecting patients to community resources related to housing, transportation, food, and activities of daily living.
  • Assist patients and their families to fully participate in healthcare services, navigate and use community services or other resources in support of their goals.
  • Participate in daily operations of the High Risk Care Team, answering incoming calls, responding to emails, and facilitating transitions of care services.
  • Teach patients and family members to connect with their care teams via technology.
Requirements:
  • At least 1 year of experience working in home-based healthcare services or senior living care settings or Emergency Medical Technician (EMT) experience;
  • Certified Medical Assistant (MA) and Basic Life Support (BLS) preferred or ability to obtain certification within 6 months of hire required;
  • Strong written and verbal communication skills; Spanish speaking is a plus;
  • A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges;
  • A proven ability to build rapport and trust with patients or customers, while delivering individualized, human-centered, customer-focused care;
  • Clinical skills in performing basic vitals and clinical assessments preferred;
  • Experience facilitating access to community resources or social service agencies preferred;
  • Proficiency in computer technology and video platforms such as typing, navigating the internet, meeting over zoom, and using multiple software systems simultaneously; experience with Mac iOS, Google Suite preferred;
  • Must have access to an independent means of safe and reliable transportation as well as the ability to travel house to house throughout the day; mileage will be reimbursed at the IRS/Federally established rate.
Compensation Package

The base hourly compensation range for this role is $21.25 to $22.25 per hour. Based on job-related knowledge and skill set, depth of experience, certifications, and/or degrees, actual compensation packages may vary.

This is a full-time role with shifts generally taking place between 8:00am-5:00pm (Monday through Friday) based in Atlanta, Georgia. Flexibility to travel throughout the day to patient homes in the Atlanta area and work in a variety of Atlanta offices based on assignment.
