Role Overview: As a Client Care Coordinator (CCC), you will be an integral part of an interdisciplinary team (IDT) alongside a Registered Nurse Care Manager, dedicated to serving members of Lakeland Care Incorporated (LCI), including the elderly, adults with physical disabilities, and those with intellectual or developmental disabilities.
The CCC is responsible for managing care and coordinating services for LCI members.
Through a thorough assessment of each member's needs and desired outcomes, the CCC will arrange for the necessary services and supports.
The IDT will oversee the delivery of services in accordance with the member-centered plan, adhering to LCI policies and Department of Health Services (DHS) contract requirements.
Field Responsibilities: This position involves traveling within the community to visit members.Engage in home visits and care conferences with members, their support systems, and service providers.
Collaborate with the RN Care Manager to ensure seamless coordination of acute and primary care services, as well as follow-up care.
Conduct comprehensive, strengths-based assessments of members' needs, outcomes, and risks; implement strategies to mitigate risks while promoting health, safety, and independence, all while respecting members' rights to appeal and grieve.
Maintain accurate member records in compliance with DHS contracts and LCI policies.Build and nurture effective working relationships with RN Care Manager partners and various departments/stakeholders.
Participate in team meetings and ongoing training to stay informed about policies, procedures, and relevant state and federal regulations.

Qualifications: Candidates should possess a Certified Social Worker license in the State of Wisconsin with at least one year of experience working with family care target populations, or a four-year degree in Human Services or a related field with similar experience. Alternatively, a four-year degree in another area combined with three years of relevant experience is acceptable.
Proficiency in computer usage, including typing and data entry, is required.
