About the Job

We are seeking a dedicated and compassionate RN to join our Home Health team at Providence Medical Center in Medford, Oregon. As a Home Health Nurse, you will provide high-quality patient care and support to individuals in their homes, focusing on restorative care, longitudinal care, and end-of-life care.

Key Responsibilities:

  • Provide comprehensive patient assessments and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless transitions and continuity of care
  • Conduct home visits to deliver medical care, educate patients and families, and address concerns
  • Document patient progress and outcomes in accordance with regulatory requirements

Requirements:

  • Current RN license in Oregon
  • 1 year of experience in medical/surgical or home health setting
  • National Provider BLS certification within 30 days of hire
