About the Role

CHPW is seeking a highly skilled and compassionate Case Manager I to join our team. As a key member of our care coordination team, you will be responsible for delivering exceptional case management services to our members, ensuring they receive the best possible care and support.

Key Responsibilities
  • Assess and evaluate member needs, identifying opportunities for care coordination and resource utilization.
  • Develop and implement personalized care plans, working collaboratively with members, families, caregivers, and healthcare providers.
  • Coordinate access to community resources, services, and support, ensuring seamless transitions and optimal outcomes.
  • Provide education and support to members and their families, empowering them to manage their health and well-being.
  • Collaborate with interdisciplinary care teams to ensure comprehensive and coordinated care.
  • Monitor and evaluate member progress, making adjustments to care plans as needed.
Requirements
  • Bachelor's degree in nursing, social work, or related field (or equivalent experience).
  • Current, unrestricted license in the State of Washington as a registered nurse (RN), social worker (LSWAA, LSWAIC), or mental health professional (LMHC, LMHP, LMFT).
  • Minimum one year of case management, home health, or discharge planning experience (or equivalent combination of education and experience).
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Proficiency in Microsoft Office products and ability to learn new software applications.
Preferred Qualifications
  • Case management certification.
  • Bilingual or multilingual skills.
  • Experience with managed care (Medicaid and/or Medicare Advantage) and care management software applications.
