Case Manager

About the RoleWe're seeking a dedicated and compassionate Case Manager to join our team. In this role, you'll engage with target members, conduct holistic health assessments, and develop personalized case management plans to address their unique needs and goals.
Key Responsibilities- Build relationships with members and their caregivers to understand their specific situation and needs
- Conduct initial and ongoing assessments to identify social determinants of health and cultural influences
- Develop and implement member-centric case management plans that address potential barriers and promote positive behavior change
- Monitor and evaluate the effectiveness of case management plans to ensure progress toward goals
- Facilitate communication and coordination among the healthcare team to maximize delivery of appropriate care
- A supportive and safe environment where clinicians can work to the top of their licensure
- Extensive training, including initial and ongoing motivational interviewing skill development
- Support in earning certifications, such as CCM, CCP, and ICM
- Paid licensure renewal and reimbursement for CCM exam fees and renewal
- Paid membership in the National Case Management Society of America (CMSA) and local state chapters
- Registered Nurse (RN) current state license
- 3 years' experience with direct patient/clinical care post-licensure
- High-speed internet connection (at least 20 Mbps)
- Prior experience as a Case Manager and Certified Case Manager (CCM) certification
- Chemical Dependency Professional (CDP) certification
- Chronic Care Professional certification (CCP)
- LEAN training
- Advanced Motivational Interviewing skills
