About the Role

We are seeking a highly skilled and compassionate Patient Care Manager to lead our hospice site team. As a key member of our clinical leadership team, you will be responsible for ensuring the delivery of high-quality patient care and services.

Key Responsibilities
  • Manage the day-to-day operations of the hospice site, including clinical and administrative functions.
  • Oversee the provision of quality care to patients, ensuring compliance with regulatory guidelines and company policies.
  • Respond to phone referrals and inquiries in a timely and professional manner.
  • Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Registered Nurse with a minimum of Associate's Degree in Nursing required.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of requested employment.
