Job Title: Homecare RN

Southminster is seeking a skilled and compassionate Homecare RN to join our team. As a Homecare RN, you will play a vital role in providing high-quality care to our clients in the comfort of their own homes.

Responsibilities:
  • Coordinate client care and ensure continuity and quality of care
  • Conduct initial and quarterly assessments for home care clients
  • Ensure staff understanding of and compliance with agency policies and procedures
  • Review documentation and perform in-home supervisor visits
  • Communicate with physicians, staff, family, and clients
  • Complete chart audit and discharge summaries for home care clients
  • Back up to the Clinic Nurse Manager for assistance and coverage
  • Perform direct and individualized nursing care to an assigned group of clients
  • Participate in planning, implementing, and evaluating care plans and nursing care
  • Perform and/or assist with treatments, therapies, and procedures
  • Administer and record oral, intramuscular, and subcutaneous medications in accordance with home care services policies, procedures, and physician orders
  • Write initial nursing assessments upon admission, assess clients' conditions, and assist with the development of individual care plans for clients
  • Document observations, treatments, client response to treatment, and nursing care. Complete periodic assessments
  • Assist in the development, updating of care plans, and IDCP process
  • Give shift report, document on 24-hour report, process physician orders, and assist with orientation of new employees
  • Participate in continuing education, staff meetings, and other required in-service training
  • Support and adhere to administrative and nursing policies and procedures
  • Know and be able to initiate fire and disaster plans. Recognize and respond to client emergencies with appropriate measures
  • Utilize resources, materials, and equipment in a safe and efficient manner
  • Demonstrate dependability by being punctual; maintain scheduled attendance
  • Support and enforce safety and infection control policies and procedures
  • Conform to facility dress code
  • Perform routine housekeeping duties, including straightening and maintaining service areas and resident rooms
  • Clean, disinfect, and maintain resident care equipment as assigned
  • Complete all other duties as assigned by supervisor
Qualifications:
  • Current license to practice as a Registered Nurse or Licensed Practical Nurse in North Carolina
  • Geriatric experience preferred
  • Knowledge of federal and state guidelines for home care and long-term senior care
  • Understanding of nursing care and knowledge of human needs and emotions
  • Ability to communicate effectively with others verbally and in writing
