Job Summary

We are seeking a skilled Public Health Nurse II to join our team at Craven County Health Department. As a key member of our primary care clinics, you will provide case management and assurance of health services to adult and pediatric patients.

Key Responsibilities
  • Provide case management services to assigned clients, utilizing nursing expertise to identify and prioritize patient problems.
  • Collaborate with clients, physicians, and other providers to develop comprehensive care plans.
  • Communicate effectively with clients and healthcare teams to ensure seamless care coordination.
  • Assist in pediatric and adult primary care clinics, performing tasks such as health histories, lab work, and medication administration.
  • Act as a client advocate, identifying and addressing community resources to support client needs.
  • Participate in quality improvement activities, using data to inform practice changes and improve patient outcomes.
Requirements
  • Graduation from an accredited school of professional nursing and one year of professional nursing experience.
  • Current license to practice as a Registered Nurse in North Carolina.
  • CPR certification.
