Job Title: Pharmacist

Job Overview

As a key member of the healthcare team, the Pharmacist delivers comprehensive pharmaceutical care to patients in long-term care settings. This role is pivotal in ensuring the highest standards of medication safety and efficacy.

Key Responsibilities
  • Review and assess physician medication orders for appropriateness, dosage, potential drug interactions, and routes of administration, suggesting modifications to therapy when necessary.
  • Maintain a precise patient medication database, ensuring that all laboratory tests are correctly ordered and evaluated.
  • Document all relevant communications in the patient's medical record to ensure continuity of care.
  • Implement and uphold procedures for the safe use, storage, and disposal of pharmaceuticals, adhering to all applicable federal and state regulations.
  • Support healthcare staff with drug procurement, maintenance, and administration processes.
Essential Skills
  • Valid pharmacist licensure.
  • In-depth knowledge of pharmaceutical practices in long-term care.
  • Expertise in evaluating and recommending medication therapies.
  • Experience with protocols for medication safety and storage.
  • Proficient in maintaining accurate patient records.

Job Type

This position is offered on a contract basis.
Work Environment

The role requires working overnight shifts, with a schedule that includes 7 consecutive days on followed by 7 days off.
