Job Title: Nurse Practitioner - Resident Care

At Sunrise Senior Living, we are seeking a skilled and compassionate Nurse Practitioner to join our team as a Resident Care Specialist. As a key member of our care team, you will play a vital role in ensuring the highest quality of life for our residents.

Responsibilities:

  • Conduct thorough assessments of residents' needs and develop personalized care plans to promote their overall well-being.
  • Collaborate with our interdisciplinary team to ensure seamless care delivery and effective communication with residents and their families.
  • Monitor and report any changes in residents' health status, and implement interventions as needed to prevent hospitalizations and promote recovery.
  • Provide education and support to residents and their families on health-related topics, including disease management, medication adherence, and healthy lifestyle choices.
  • Participate in quality improvement initiatives to enhance the overall quality of care and services provided by our community.

Requirements:

  • Graduate of an accredited nursing program with a current state license as a Nurse Practitioner.
  • Minimum of one year of experience in long-term care, home health, or a related field.
  • Strong clinical skills, with the ability to assess and manage complex medical conditions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to providing exceptional care and service.
