Job Title: Registered Nurse - Home Care

Aveanna Healthcare LLC is seeking a compassionate and skilled Registered Nurse to provide high-quality nursing care to patients in their homes and schools. As a key member of our team, you will be responsible for delivering patient-centered care, collaborating with healthcare professionals, and promoting health and wellness in the community.

Responsibilities:
  • Provide skilled nursing care to patients in their homes and schools, adhering to physician orders and care plans.
  • Conduct thorough patient assessments, identify care needs, and develop and implement individualized care plans.
  • Administer medications, treatments, and therapies as prescribed by physicians, and document all care activities in a timely manner.
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive patient care.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Maintain accurate and up-to-date patient records, adhering to Aveanna's policies and procedures.
Requirements:
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
