Description of Job:
The Behavioral Health Care Coordinator will be responsible for providing fully integrated behavioral health medical services to children/youth in the child welfare system. The position is based in Brooklyn, Queens & Long Island and requires a strong commitment to the mission and programs of SCO Family of Services. The ideal candidate will have excellent communications skills, both oral and written, a strong sense of empathy towards our populations served, and the ability to work varying schedules that may cause them to work in excess of their regularly scheduled hours or weekends if necessary.

Salary: $80,000 - $105,000 per year

Duties:
• Assess risk factors for mental illness and broader wellness evaluations
• Screen patients for physical illness and analyze explanations for psychiatric illness
• Provide individual, group or family counseling sessions, and write prescriptions for psychotropic medications under the supervision of psychiatrists and when state regulations allow.
• Manage caseload and documentation.
• Facilitate communication between psychiatric services, programs and care givers.
• Initiate, monitor and recommend medications and treatment changes.
