Position Overview:
The Clinical Case Manager for Emergency Services will oversee the transitional care for a designated number of cases by coordinating, managing, and monitoring resources to ensure optimal clinical outcomes, efficient service utilization, and financial objectives.
This role is crucial in supervising the discharge process for all patients within the emergency department and plays a vital part in enhancing patient flow throughout the healthcare facility.
Essential Qualifications:
Must possess a valid NYS RN license (or be eligible), along with a Bachelor's degree in Nursing or a related healthcare discipline, and have at least three (3) years of experience in acute care nursing or case management in acute, transitional, or chronic care settings. Alternatively, an Associate's degree in Nursing with five (5) years of relevant experience is acceptable.
*If holding an Associate's degree, enrollment in an accredited BSN program with an expected graduation within two years is required. Temporary appointment until BSN is obtained. CPR-BLS certification must be completed by the time of appointment.
Desirable Qualifications:
A Master's degree in Nursing or a related healthcare field is preferred, along with prior experience in acute care case management, familiarity with Interqual Criteria, and/or Quality Improvement/Utilization Review experience. Experience in the emergency department is also preferred; PRI certification is an advantage.
Work Schedule:
Monday through Friday, with occasional weekend and holiday shifts as needed.
