Emergency Department RN Clinical Case Manager

The Emergency Department RN Clinical Case Manager plays a vital role in facilitating transitional care for patients by managing, coordinating, and monitoring resources to achieve optimal clinical outcomes, utilization of services, and financial goals.
This position oversees the discharge process for all patients within the service area and is an integral team member in efforts to facilitate throughput throughout the hospital system.
Key Responsibilities:
• Manage and coordinate resources to achieve optimal clinical outcomes, utilization of services, and financial goals.
• Oversee the discharge process for all patients within the service area.
• Collaborate with the healthcare team to facilitate transitional care and ensure seamless patient flow.
• Monitor and analyze data to identify areas for improvement and implement changes to optimize patient care and outcomes.
• Develop and implement case management plans to address the unique needs of patients.
• Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care and support.
Requirements:
NYS licensed RN (or eligible), Bachelor's degree in Nursing or a healthcare-related field, plus a minimum of three (3) years acute care RN or acute care, transitional care, or chronic care RN/case management experience OR Associates degree in Nursing and five (5) years acute care RN or acute care, transitional care, or chronic care RN/case management experience required.
*If Associate in Nursing, must be enrolled in an accredited BSN program with an anticipated graduation date within the next 2 years.
Will be appointed temporary until receipt of BSN. CPR-BLS certification is required by the time of appointment.Preferred Qualifications:
Master's degree in Nursing or healthcare-related field and prior acute care case management experience, knowledge of Interqual Criteria, and/or QI/UR experience preferred.
