About the Role

We are seeking a highly skilled and compassionate Medical Care Provider to join our team at Abode Care Partners. As a Nurse Practitioner, you will play a vital role in providing evidence-based medical care to seniors and patients with complex needs.

Our ideal candidate will have a strong knowledge base of HIPAA and OSHA regulations, as well as proficiency in medical instruments and equipment. You will be responsible for examining and treating chronic and urgent episodic illnesses, minor injuries, and other health issues. Additionally, you will collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive patient records.

Key Responsibilities
  • Perform evidence-based medical care by planning, directing, and evaluating patient medical needs
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other health issues
  • Order and interpret laboratory and diagnostic testing as needed
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining comprehensive patient records
Requirements
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
