Social Worker-Full Time-North Hospital

We are seeking a highly skilled and compassionate Social Worker to join our team at Quality Talent Group. As a Social Worker, you will play a critical role in coordinating patient-centered care and service delivery to facilitate optimal transitions and progression in care.
Key Responsibilities- Coordinate and collaborate with patient, caregiver, physician, and other members of the healthcare management team to ensure seamless care transitions.
- Develop and implement effective transition plans to promote efficient utilization of resources and reduce readmission risks.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
- Collaborate with interdisciplinary teams to identify and address social determinants of health and develop strategies to improve patient outcomes.
- Monitor and evaluate the effectiveness of care transitions and make recommendations for process improvements.
- Master's Degree in Social Work (MSW) or related field.
- 5-7 years of experience in a licensed social work role, with 2 years in an inpatient/acute care setting.
- Licensed Clinical Social Worker (LCSW) or Licensed Medical Social Worker (LMSW) certification.
- ACM/CCM certification preferred.
