Job Title: Medicaid Service Coordinator

Job Summary:

We are seeking a highly skilled Medicaid Service Coordinator to join our team at TalentPlug LLC. As a Medicaid Service Coordinator, you will be responsible for initiating, assisting, and managing Medicaid applications for prospective participants, conducting home visits to assist participants with completing the Medi-Cal application process, and maintaining accurate documentation and record participant enrollment information.

Key Responsibilities:

  • Initiate, assist, and manage Medicaid applications for prospective participants
  • Conduct home visits to assist participants with completing the Medi-Cal application process
  • Maintain accurate documentation and record participant enrollment information
  • Adhere strictly to DHCS and CMS regulations and policies
  • Collaborate with Finance Department for data analysis and reporting
  • Monitor and maintain participants' Medicaid eligibility
  • Provide community resources to prospective participants and families/caregivers
  • Collaborate with government specialists for required documentation

Requirements:

  • Associate degree or higher
  • 2+ years of experience in ALL of the following:
  • Medicaid Eligibility
  • Case Management
  • Clinical Documentation

Preferred Qualifications:

  • 1+ years of experience in ANY of the following:
  • In-Home Visits
  • Medi-Cal Process
  • PACE Model Experience
  • Working with Elderly
