Position Summary
The Hospice Care Assistant plays a vital role in delivering personal care services tailored to the unique needs of each patient. This position is essential in fostering a supportive environment that promotes the well-being of both patients and their families.
Key Responsibilities
- Deliver personalized care to patients based on their specific needs and preferences.
- Communicate effectively with patients and families, ensuring a respectful and confidential approach.
- Maintain a safe environment by using equipment properly and attending to safety protocols.
- Assist with daily living activities, including bathing, grooming, and dressing.
- Prepare nutritious meals and assist with feeding as needed.
- Monitor and document vital signs, reporting any significant changes to the healthcare team.
- Support mobility and exercise routines as directed by healthcare professionals.
- Provide companionship and emotional support to patients and their families.
- Ensure cleanliness and organization of the patient's living space.
- Participate in ongoing training and quality improvement initiatives.
Qualifications
- Completion of state-required training and certification.
- Prior experience in personal care or a related field is preferred.
- Ability to follow written instructions and maintain accurate documentation.
- Strong interpersonal skills and a compassionate demeanor.
- Current CPR certification is required.
- Ability to work independently while being part of a collaborative team.
