Qualifications:
- Bachelor's or Master's Degree required; Doctorate preferred
- Must be a licensed Physical Therapist in the practicing state
- Completion of state-mandated continuing education units
- Valid CPR certification
- Familiarity with EMR software is advantageous
- Strong communication skills, both oral and written
- Excellent clinical judgment and commitment to patient success
- Basic computer proficiency
- Ability to work independently and as part of a team
- Maintain a professional demeanor at all times.
Responsibilities Include:
- Collecting and documenting patient care data in accordance with regulatory standards
- Conducting initial evaluations, daily progress notes, and discharge planning
- Preparing for evaluations by gathering patient history and understanding diagnoses
- Administering tests and synthesizing results to establish treatment goals and plans
- Collaborating with interdisciplinary teams and adjusting care plans as necessary
- Providing education and training to patients and their support systems
- Overseeing the treatment and care plans for your patient caseload
- Ensuring interventions are safe, effective, and compliant with JAG standards
- Supervising support staff and participating in community outreach initiatives.
