Bilingual Care Coordinator

Zrpath Behavioral Health Services is dedicated to assisting children and families facing behavioral health issues. As a Care Specialist, you will support clients aged birth to 17 with behavioral health diagnoses in accessing essential community resources. You will collaborate with guardians, caregivers, and other professionals to create comprehensive care plans tailored to each client’s needs.
Key Responsibilities
- Work Schedule: Monday – Friday – 8:30am-5pm
- Evaluate clients' physical and mental health, preferences, and capabilities.
- Design personalized care strategies.
- Engage with clients, their support networks, and healthcare providers to implement care plans effectively.
- Document clients' progress, including referrals, home visits, and significant interactions.
- Regularly assess clients' progress and modify care plans as necessary to enhance outcomes.
- Follow up with clients post-discharge to ensure satisfaction with services and ongoing well-being.
- Participate in and facilitate CFT meetings for your assigned clients.

Qualifications and Skills
Applicants must be at least 21 years old.
A minimum of 2 years of experience in the behavioral health sector, specifically working with youth and families, is required.
Must hold a valid Arizona driver's license and be insurable under the agency's automobile policy.
Possession of a valid Arizona Fingerprint Clearance Card or the ability to obtain one is necessary.
Proficiency in computer applications, including Outlook and Microsoft Office (Word, Excel), is essential.
Fluency in both Spanish and English is required.
Compensation: $36,000-$40,000 annually.
