Summary

We are hiring a Physical Therapist to work in Senior Living.

Responsibilities
  • Evaluate patients to determine their rehab potential, identify their needs, establish an individualized treatment plan of care, and set goals to be used in the team plan of care.
  • Set initial short-term goals and long-term goals.
  • Use equipment in a proper and safe manner, maintain equipment inspection, maintenance, and cleaning logs, and report broken or faulty equipment.
  • Maintain equipment logs for required equipment in the outpatient clinic.
  • Follow the medical regimen developed for the patient. Be aware of and adhere to precautions (THR, TKR, Cardiac, Wound, etc.).
  • Use safe treatment techniques: transfers, positioning, feeding, splinting, paraffin bath, cryotherapy, ultrasound, electrical stimulation, etc.
  • Complete a discharge summary on each patient on their final visit to outpatient PT.


Education and Experience
  • Current Physical Therapist licensure in the state of practice.
  • Current CPR certification required.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle, or public transportation.
