Job Summary

We are looking for a highly motivated and dedicated Home Health Care Professional to provide high-quality patient care in the comfort of their own homes. As a key member of our interdisciplinary team, you will work closely with healthcare professionals to develop and implement personalized care plans that meet the unique needs of each patient.

In this role, you will be responsible for providing various aspects of patient care, including bathing, grooming, and feeding. You will also assist with activities of daily living, such as toileting, transferring, and ambulating. Additionally, you will maintain accurate records and communicate effectively with patients, families, and healthcare providers.

Key Responsibilities
  • Provide high-quality patient care, including bathing, grooming, and feeding, as well as assisting with activities of daily living.
  • Develop and implement personalized care plans in collaboration with the interdisciplinary team.
  • Maintain accurate and detailed records of patient care activities.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Work collaboratively with other healthcare professionals to address complex patient needs.
Requirements

To be successful in this role, you will need:

  • A minimum of one year of experience in a clinical setting, preferably as a certified nurse aide or home health aide.
  • Current certification in hospice and palliative care, preferred.
  • A high school diploma or equivalent, required.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking skills.
Benefits

Hospice Buffalo offers a competitive salary range of $45,000 - $60,000 per year, depending on experience, as well as a comprehensive benefits package, including:
