Position Summary: We are in search of a proficient and compassionate Family Nurse Practitioner to become a vital part of our healthcare team. The ideal candidate will be responsible for conducting thorough patient assessments, offering medical consultations, and working alongside our diverse team to create and execute tailored treatment strategies.
Core Responsibilities:
- Perform detailed patient assessments and physical examinations.
- Provide medical consultations and evaluations for individuals enrolled in our rehabilitation programs.
- Collaborate with the healthcare team to design, implement, and assess personalized treatment plans.
- Track and document patient progress, modifying treatment strategies as needed.
- Educate and counsel patients and their families on treatment alternatives, medications, and lifestyle changes.
- Facilitate care coordination with physicians, mental health specialists, and other healthcare professionals.
- Ensure accurate and current patient documentation in accordance with regulatory requirements.
- Engage in quality improvement projects and ongoing professional development activities.
- Master's degree in Nursing with a certification as a Family Nurse Practitioner.
- Valid state licensure as a Family Nurse Practitioner.
- A minimum of 2 years of clinical experience, ideally in a rehabilitation or mental health context.
- In-depth knowledge of addiction medicine and evidence-based treatment methodologies.
- Exceptional communication and interpersonal abilities.
- Capability to work effectively within a multidisciplinary team framework.
- Strong organizational and time management skills.
- Availability to work in an office environment 2-3 times per week.
