About the Role

As a Registered Nurse with LHC Group, you will be responsible for providing high-quality, patient-centered care in the comfort of our patients' homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities

  • Conduct comprehensive assessments of patients to identify their physical, emotional, and social needs
  • Develop and implement individualized care plans that address the patient's needs and goals
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication
  • Document patient progress and outcomes in a timely and accurate manner

Requirements

  • Current RN licensure in the state of MA
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
