Job Opportunity

Pure Healthcare is seeking a dedicated professional to fill the role of Community Health Worker and Outreach Coordinator.

Key Responsibilities:

  • Collaborate with the clinical team to provide comprehensive care to patients with serious illnesses.
  • Deliver high-quality supportive care services to patients, their families, and community providers.

Requirements:

  • Minimum 1-year healthcare experience and understanding of person-centered care philosophy, Care Coordination, and/or Managed Care setting.
  • Healthcare background required.
  • LPN license or experience.
  • Ability to empathize with patients and understand their challenges.
  • Experience assisting patients with Social Determinants of Health (SDoH) needs.
  • Demonstrated success in working as part of a multi-disciplinary team, including speaking and working with physicians, clinical healthcare providers, and registered nurses.
