CareStar Assessment Specialist

Are you a motivated individual looking for a challenging role in healthcare? CareStar is seeking an Assessment Specialist to join our Clinical department. As a key member of our team, you will conduct program assessments using our proprietary tools and processes.

Responsibilities:
  • Conduct home-based assessments using our program and CareStar-identified tools and processes.
  • Manage a caseload as needed, working independently to ensure timely and accurate assessments.
  • Collaborate with our team to develop and implement assessment strategies that meet the needs of our clients.
  • Stay up-to-date on industry developments and best practices in home and community-based services.
Requirements:
  • Active license as RN, LSW, or LISW is preferred.
  • 12 months of experience in home and community-based services within the past 5 years.
  • Knowledge of Medicare, Medicaid, home health care, and Ohio Home Care waiver programs is desirable.
