Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Mary T Inc. as a Group Home Care Specialist. In this role, you will be responsible for providing high-quality nursing care to individuals in our group homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Conduct focused assessments of health status and report changes and responses to interventions to registered nurses or licensed health providers.
  • Participate in the development and modification of care plans with other healthcare providers.
  • Determine and implement appropriate interventions within a nursing plan of care, as delegated or ordered by a registered nurse or licensed healthcare provider.
  • Assign and monitor nursing tasks or activities to unlicensed supportive personnel, promoting a safe and therapeutic environment.
  • Assist in the evaluation of responses to interventions and collaborate with other healthcare providers.
  • Provide health care information to individuals, staff, or other interdisciplinary team members.
  • Provide input into the development of policies and procedures.
  • Accountability for the quality of care delivered, recognizing the limits of knowledge and experience.
Requirements
  • Active LPN license in Minnesota.
  • Experience in an ICF licensed group home is preferred.
  • Valid driver's license with a good driving record.
  • Ability to pass a background check.
  • Strong verbal and written communication skills.
  • Basic computer skills.
