Job Description

As a Clinical Care Coordinator, you will play a vital role in managing patient cases and ensuring seamless care transitions. Your primary responsibility will be to coordinate patient care activities, including developing and implementing individualized care plans.

Key Responsibilities
  • Develop and implement comprehensive care plans that address patient needs and goals.
  • Coordinate with interdisciplinary teams to ensure timely and effective care delivery.
  • Provide education and support to patients and their families to promote health and wellness.
Requirements

To be successful in this role, you must have a strong background in nursing and excellent communication skills. A Bachelor's degree in Nursing is required, and experience in case management or a related field is preferred.
