Clinical Care Coordinator

About the RoleThis is a full-time position working Monday through Friday, 8am-5pm. The ideal candidate will have a strong understanding of clinical healthcare practices, preferably in a primary care setting.
The Clinical Care Coordinator is responsible for preparing patients for visits by completing advanced standard room processes, including taking vital signs, reviewing patient history and allergies, and completing medication reconciliations.
You will also work closely with providers and clinical care teams to monitor and coordinate delivery of the care plan, scheduling follow-up appointments as needed. In addition, you may assist with in-office testing, administer vaccines and other injections within your scope of practice, and complete documentation in Epic for yearly preventative visits.
Requirements- Education: High School Diploma or GED
- Certification: Must obtain BLS certification within 90 days of hire
- Preferred Experience: 6 months of clinical healthcare experience, preferably in a primary care office
