Job Title: Patient Care Coordinator

Job Summary: We are seeking a dedicated Registered Nurse to join our team as a Home Health Patient Care Coordinator. The ideal candidate will possess excellent clinical skills, effective communication abilities, and a passion for delivering high-quality patient care.

Key Responsibilities:
  • Develop and implement individualized plans of care for patients in accordance with their unique needs and goals.
  • Collaborate with healthcare professionals to ensure comprehensive care coordination and optimal patient outcomes.
  • Maintain accurate and detailed records of patient interactions, treatment plans, and progress notes.

Requirements:
  • Bachelor's degree in Nursing (BSN) or equivalent experience.
  • Current RN licensure in the state of practice.
  • Excellent communication and interpersonal skills.

Estimated Salary Range: $65,000 - $85,000 per annum, depending on qualifications and experience.
