Responsibilities:
We are currently looking for full-time LPNs to join our residential program. The LPN operates under the supervision of a registered nurse and delivers nursing and facilitation services to designated clients.

The LPN on the Rehabilitation Team plays a vital role in implementing evidence-based treatment strategies that enhance self-care and promote physical, psychosocial, and spiritual well-being.

The LPN also provides education to clients and their families on promoting health, wellness, and medical stability.

Essential Duties & Functions

• Timely identification of individuals requiring medical nursing services or interventions, providing necessary care as needed.

• Possesses specialized knowledge and clinical skills essential for client care; delivers hands-on nursing and facilitation services using the nursing process.

• Educates families, funders, and significant others, utilizing appropriate resources to create and execute individualized teaching and discharge plans.

• Coordinates nursing care activities in collaboration with interdisciplinary team members to achieve treatment goals.

• Contributes to the nursing assessment of clients and their families, including their educational and discharge needs.

• Implements the interdisciplinary care plan within the scope of LPN practice, providing nursing care and education in collaboration with Brain Injury specialists as necessary.

• Assists in evaluating nursing care and modifying plans as needed to meet measurable objectives.

• Communicates essential medical/nursing information to team members, physicians, and other healthcare providers.

• Actively seeks current information regarding client programs and organizational updates.

• Ensures consistent implementation of strategies through team education and feedback, reviewing medical documentation as needed.

• Adheres to professional standards of treatment, licensure, and state regulations.

• Demonstrates the ability to support clients in challenging situations, providing necessary assistance to help them achieve treatment goals.

• Responsible for direct clinical care as required.

• Educates team members on strategies to support client treatment objectives.

• Participates in orientation and staff training on health-related topics.

• Prioritizes tasks to balance direct client treatment, team communication, assigned responsibilities, and documentation.

• Completes all necessary medical documentation, including progress notes and medication records.

• Documents each nursing session promptly and accurately.

• Conducts nursing assessments and evaluations within established time frames.

• Engages in collaborative problem-solving with the team to support clinical decision-making.

• Maintains current professional certification or licensing, participates in continuing education, and stays informed about new developments in the field.

Qualifications:
• PA licensure as a LPN is required.

• Experience in the rehabilitation of chronically or catastrophically ill or injured individuals is preferred.

• Demonstrated accountability and skills in analysis, problem-solving, decision-making, and time management are essential.

• Strong written and verbal communication skills with families, clients, staff, and physicians are necessary.

• A valid driver's license and responsible driving behavior must be maintained.
