We are seeking a skilled Medical Oncology Patient Care Navigator to join our team, who will play a critical role in coordinating patient care services, developing and implementing clinical guidelines, and fostering relationships with patients, families, and healthcare providers.

Key Responsibilities

  • Coordinate patient care services to ensure excellence in patient care and patient flow.
  • Develop and implement clinical guidelines to promote optimal clinical outcomes.
  • Collaborate with multidisciplinary teams to achieve common goals and improve patient care.
  • Foster relationships with patients, families, and healthcare providers to provide seamless care coordination.

Requirements

  • Graduate of an accredited school of nursing.
  • Bachelor's degree in nursing within 4 years of hire.
  • Two (2) years clinical practice experience, 5 years of service line experience preferred.

Estimated Salary Range: $65,000 - $85,000 per year, depending on experience and qualifications.
