About the Role

We are seeking a skilled RN Case Manager to join our team at Community LIFE, a program of all-inclusive care for the elderly. As a RN Case Manager, you will play a vital role in empowering older adults to remain at home while preserving their dignity, independence, and quality of life.

Job Summary

The RN Case Manager provides in-home assessments and overall care-management to a caseload of frail older adults whose goal is to continue living in their own home. This involves assessing the need for, and coordinating the delivery of, nursing and home care services. The RN Case Manager must be able to work independently and utilize critical decision-making skills, while also being able to work effectively with the interdisciplinary team approach to care for the elderly.

Responsibilities
  • Conduct in-home assessments and develop comprehensive care plans for frail older adults
  • Coordinate the delivery of nursing and home care services
  • Work collaboratively with the interdisciplinary team to ensure high-quality care
  • Provide education and support to patients and their families
  • Manage multiple priorities and work effectively in a fast-paced environment
Requirements
  • Graduate of an accredited nursing school with at least one year of experience working with a frail elderly population
  • Valid Driver's License
  • Knowledge of current nursing practice and theory, as well as the medical, social, and emotional needs of a frail, elderly population
  • Effective written and oral communication skills, as well as strong organizational and planning skills
