Job SummaryWe are seeking a skilled Registered Nurse to join our Afterhours AIM (Advanced Illness Management) Program. As a key member of our team, you will provide preventive and supportive care to palliative patients and their families, utilizing your assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life.
Responsibilities- Triage patient needs over the phone and respond to crisis calls from patients and families after hours
- Make initial nursing evaluations to determine eligibility for palliative services within 48 hours of referral
- Identify patient/family physical, psychosocial, emotional, and environmental needs and reassess as needed
- Collaborate with patients, families, attending physicians, and other members of the IDG to provide patient and family care
- Timely document patient problems, goals, interventions, and response to palliative care
- Maintain accurate and relevant clinical notes regarding patient condition
- Communicate effectively with AIM team members, clinical supervisor, and patient's PCP regarding patient status and needs
- Associates/Bachelor's degree in Nursing
- 2 years of prior skilled Registered Nurse experience
- Demonstrated ability to provide quality patient care
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
