Job Title: Registered Nurse Case Manager

We are seeking a skilled and compassionate Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire. As a Home Health RN, you will have the opportunity to provide exceptional patient care while enjoying a flexible and autonomous work environment.

Job Summary:

The RN Case Manager will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community. This is a full-time, part-time, and per diem opportunity.

Responsibilities:
  • Manage a defined caseload of patients, providing direct patient care and comprehensive nursing services.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Develop and implement individualized care plans to meet patients' unique needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and support to patients and families to promote healthy outcomes.
Requirements:
  • Valid Nursing License in the State of New Hampshire.
  • Nursing experience required.
  • Current CPR Certification required.
  • Reliable transportation.
