Job Title: Registered Nurse Case Manager

We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire. As a Home Health RN, you will have the opportunity to provide exceptional patient care while enjoying a flexible and autonomous work environment.

Job Summary:

The RN Case Manager will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community. This is a full-time, part-time, and per diem opportunity based out of our Portsmouth, NH office.

Responsibilities:
  • Manage a caseload of patients and provide direct patient care
  • Conduct comprehensive nursing assessments and develop individualized care plans
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families
  • Document patient information and maintain accurate records
Requirements:
  • Valid Nursing License in the State of New Hampshire
  • Nursing experience required
  • Current CPR Certification required
  • Reliable transportation
