Job Title: Certified Case Counselor I

Job Summary:

The Certified Case Counselor I plays a vital role in providing guidance and assistance to patients at the Peter Ho Memorial Center. This position is responsible for assessing medical, insurance, housing, social services, community, legal, financial, and other services to ensure patients receive comprehensive care.

Key Responsibilities:

  • Provide guidance and assistance to patients in assessing their needs and developing individualized service plans.
  • Support the medical case management team with completion of intakes, psychosocial assessments, and follow-up on referrals as needed.
  • Perform client advocacy and patient retention activities, treatment adherence counseling, and termination or inactivation of cases.
  • Maintain documentation in the EPIC, NJDOH system (CareWare), and Ryan White Grant reporting systems (CHAMPS).
  • Educate and consult with service providers and consumers concerning HIV disease and the process of meeting consumers' identified needs.

Requirements:

  • Minimum of one year's experience working in case management, discharge planning, or other health-related fields.
  • High School degree required; Associate degree or higher in Social Work, psychology, or other healthcare-related fields preferred.
  • Ryan White Case Management Certificate obtained within 6 months of hire is required.
  • Experience and knowledge in basic to intermediate computer skills (Microsoft Office: Word, Excel, PowerPoint, email, etc.).
