Job Overview
Oncology Formulary Oversight:
Administer the oncology formulary by assessing and integrating new treatment agents.


Identify the suitable care setting for patients undergoing oncology therapy, taking into account inpatient, outpatient, or regional campus options.

Supervise the evaluation process for off-label, non-formulary, or other non-standard applications of antineoplastic agents.

Encourage appropriate, standardized, and guideline-compliant antineoplastic use across all El Camino Health (ECH) locations where applicable.

Examine provider requests for exception orders and assess for literature support to facilitate the prior authorization process when necessary.


Engage with relevant stakeholders, including physicians and other healthcare professionals, to ensure the safe and effective administration of these agents.


Collaboration with Pharmacy Informatics Teams:
Partner closely with pharmacy informatics teams to aid in the creation and revision of Electronic Health Records (EHR) resources.


Ensure that EHR resources promote the safe and effective application of oncology medications and comply with current guidelines and best practices.


Education and Training:


Coordinate, facilitate, and deliver education, training, competency assessments, and performance improvement initiatives to pharmacists, pharmacy residents, and other healthcare professionals regarding the appropriate use of antineoplastics, as well as on best practices and the latest advancements in oncology pharmacy.

Collaborate with other disciplines to develop and implement best practices throughout the institution related to antineoplastic use.

Quality Assurance and Performance Enhancement:


Contribute to the institution's antineoplastic stewardship program by compiling data and metrics on antineoplastic use in both outpatient and inpatient environments, reporting findings to relevant stakeholders.

Monitor antineoplastic usage practices to identify areas for enhancement and implement necessary adjustments.

Establish and maintain databases and dashboards that support the work of the Antineoplastic Stewardship Program as required.

Collaborate with the Department of Quality and Patient Safety in the evaluation, mitigation, and dissemination of antineoplastic-related safety initiatives.


Stay informed on the latest developments in oncology pharmacy and ensure that pharmacy services align with current standards of care.


Contribute to the formulation and maintenance of policies and procedures to ensure the safe use of antineoplastic medications across all ECH locations, enhancing patient outcomes through safe, effective, cost-efficient, and appropriate medication therapy for patients receiving antineoplastic treatment.

Qualifications:
Doctorate degree in pharmacy (Pharm.D.) from an ACPE-accredited college of pharmacy.

A one (1) year accredited PGY1 (Post Graduate Year) pharmacy residency.


A one (1) year PGY2 Pharmacy Residency in Oncology/Hematology OR BCOP certification with a minimum of 2 years of oncology experience, including pharmacy regulatory compliance and compiling medication use evaluation (MUE) data or equivalent quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.

License/Certification/Registration Requirements:
Valid California state pharmacist license with no disciplinary actions against the licensee that would prevent the practice of pharmacy.

Valid CA driver's license.

Valid NPI (National Provider Identifier) number issued by the Center for Medicare and Medicaid Services (CMS).

Patient Demographics:
This position is responsible for the assessment, treatment, and care of adolescent, adult, and geriatric patients.

Salary Range:
$112.64 USD Hourly
