- Support patients with exercises, ambulation, and transfer activities, utilizing assistive devices as needed.
- Prepare and serve meals, adhering to special diets and dietary restrictions.
- Remind patients to self-administer medications and monitor vital signs.
- Identify changes in patient condition and notify the registered case manager.
- Provide basic homemaking and laundry assistance to patients.
- Follow established policies and procedures for patient care activities.
- Maintain a safe and orderly work environment in patients' homes.
- Certified Nursing Assistant (CNA) certification or completion of a 75-hour nursing course.
- Working knowledge of meal preparation, special diets, and sanitary kitchen practices.
- Ability to competently take temperatures, heart rates, and respiratory rates.
- Minimum six months of experience in a healthcare setting.
This role requires significant manual skills, motor coordination, and finger dexterity. You will work in patients' homes, exposing you to various environments and conditions.
