About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Adoration Home Health and Hospice. As a LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Provide routine care and general observation of clients' health status
  • Assist Registered Nurses and physicians in treatments and communicate with clients and their families/guardians
  • Report changes in condition and/or additional client needs to supervisors
  • Administer medications, feedings, oxygen, and ostomy care as needed
  • Monitor clients' vital statistics and report abnormal findings
  • Document nursing actions and communicate with family, case manager, and physicians
  • Coordinate and order supplies, medications, and feedings as needed
  • Ensure all patient needs are met in accordance with physician instructions
  • Monitor incidents and ensure incident reports are completed timely and accurately
Requirements
  • LPN/LPT/LVN degree from an accredited college of nursing
  • Current unrestricted registration and license in the state of practice
  • Valid driver's license, acceptable driving record, and proof of car insurance
  • CPR professional certification
  • Experience in administering medications
  • 1+ years of nursing experience preferred
  • Home health care experience preferred
