Job SummaryThe Clinical Nurse Specialist, MDS Nurse will work closely with our interdisciplinary team to ensure the delivery of high-quality patient care. This role will be responsible for conducting assessments and care plan coordination for residents assigned to their care.
Key Responsibilities- Assessment and Care Planning
- Conduct thorough assessments of residents through physical examination, interview, and chart review.
- Develop and implement individualized care plans that address the unique needs of each resident.
- Collaboration and Communication
- Work closely with care team members, including physicians, nurses, social services, therapy, dietary, and activity staff to ensure comprehensive care.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless care transitions.
- Regulatory Compliance
- Ensure compliance with regulatory requirements, including the Resident Assessment Instrument (RAI) process and the Prospective Payment Process (PPS).
- Stay up-to-date with the latest industry standards and best practices.
- Leadership and Professional Development
- Participate in ongoing education and training to enhance clinical knowledge and skills.
- Contribute to the development of policies and procedures that promote high-quality patient care.
- Education and Certification
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) degree.
- AANC certification (AANC) or RAC-CT certification preferred.
- Experience
- Minimum 2 years of experience as an MDS Nurse or Clinical Nurse Specialist.
- Experience with the RAI process and PPS strongly preferred.
