Job Summary

The Clinical Nurse Specialist, MDS Nurse will work closely with our interdisciplinary team to ensure the delivery of high-quality patient care. This role will be responsible for conducting assessments and care plan coordination for residents assigned to their care.

Key Responsibilities
  • Assessment and Care Planning
    • Conduct thorough assessments of residents through physical examination, interview, and chart review.
    • Develop and implement individualized care plans that address the unique needs of each resident.
  • Collaboration and Communication
    • Work closely with care team members, including physicians, nurses, social services, therapy, dietary, and activity staff to ensure comprehensive care.
    • Communicate effectively with residents, families, and healthcare providers to ensure seamless care transitions.
  • Regulatory Compliance
    • Ensure compliance with regulatory requirements, including the Resident Assessment Instrument (RAI) process and the Prospective Payment Process (PPS).
    • Stay up-to-date with the latest industry standards and best practices.
  • Leadership and Professional Development
    • Participate in ongoing education and training to enhance clinical knowledge and skills.
    • Contribute to the development of policies and procedures that promote high-quality patient care.
Requirements
  • Education and Certification
    • Registered Nurse (RN) or Licensed Practical Nurse (LPN) degree.
    • AANC certification (AANC) or RAC-CT certification preferred.
  • Experience
    • Minimum 2 years of experience as an MDS Nurse or Clinical Nurse Specialist.
    • Experience with the RAI process and PPS strongly preferred.
