Care Management Registered Nurse

We are seeking a dedicated and compassionate Registered Nurse to join our team at Oceanside Center in Hampton, NH. As a Part Time RN, you will have the opportunity to make a meaningful difference in the lives of our patients and residents.
About the RoleThis Part Time RN position is available on Evening and Night Shifts, offering a unique opportunity to work in a dynamic and fast-paced environment. You will be responsible for providing high-quality patient care, collaborating with the nursing team and other disciplines to develop effective plans of care, and communicating patient information with assigned staff and between shifts.
Responsibilities- Perform nursing assessments and develop effective plans of care in collaboration with the nursing team and other disciplines.
- Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented.
- Communicate patient information with assigned staff and between shifts.
- Establish realistic, measurable short and long-term patient goals with interventions put in place as needs arise to minimize rehospitalizations and continued evaluation of outcomes.
- Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- CPR Certification is required.
- One to three years of experience working as a RN in a related setting is preferred.
