Job Summary

The RN Care Manager is a key member of the inpatient care team, responsible for coordinating patient care services and ensuring seamless transitions from hospital to home. This role requires strong clinical knowledge, excellent communication skills, and the ability to work collaboratively with interdisciplinary teams.

Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care
  • Coordinate discharge planning and post-discharge care to ensure smooth transitions
  • Monitor and evaluate patient outcomes to identify areas for improvement
  • Provide education and support to patients and families to promote self-care and independence
Requirements
  • Minimum 2 years of nursing experience in a pediatric acute care or ambulatory setting
  • Strong clinical knowledge and skills in pediatric nursing
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment with multiple priorities
  • Basic Life Support (BLS) certification and Pediatric Advanced Life Support (PALS) certification
Preferred Qualifications
  • 3 years of nursing experience in an emergency department and/or case management, care coordination or transition of care
  • Previous pediatric experience and/or management, and/or charge nurse role acceptable
  • Certification in Utilization Review and/or Case Management
