Job Summary

The RN Care Manager is a vital member of the inpatient care team, responsible for delivering care coordination services to patients admitted to the NICU. This role requires strong clinical knowledge, excellent communication skills, and the ability to work collaboratively with the interdisciplinary team to ensure efficient and cost-effective care. The ideal candidate will have a minimum of two years of nursing experience in a pediatric acute care or ambulatory setting, with a preference for NICU experience. A Bachelor's degree from an accredited university is required, and a Master's degree in a healthcare-related field is preferred. Certification in Utilization Review and/or Case Management is also desirable. The RN Care Manager will be responsible for reviewing the formulated plan of care, facilitating patient care to care providers post-discharge, and promoting efficient use of healthcare resources. This role requires the ability to work effectively under pressure, with excellent interpersonal communication, conflict resolution, problem-solving, and negotiation skills. The successful candidate will be proficient in Microsoft Office and have knowledge of CCS/Medi-Cal regulations and requirements. Bilingual in English/Spanish is a plus.

Responsibilities

Requirements

• Bachelor's degree from an accredited university
